Description

HHMC & FRC is seeking a Family Practice Physician who will see patients independently, in collaboration with mid-level providers as well as patients referred by other physicians. A major purpose of this role will be to fully participate in the development of the integrated healthcare model. 

Requirements

Essential Functions:

Provide complete, comprehensive, family-oriented outpatient care for patients throughout the life cycle, including health examinations and treatment for medical conditions, and any problem detected.

• Manage acute and chronic illnesses, develop/coordinate a plan of care, and determine the need for case conference.
• Make appropriate referrals to both the internal disciplines of the clinic and/or external agencies for additional services required.
• Work with the Behavioral Health team to provide continuity of care for the behavioral health population.
• Provide clinic consultation to mid-level providers as needed.
• Provide accurate, timely documentation in the patient’s medical record of patient contacts, case planning and plan of care.

Qualifications:

• Current California Medical License (Required)
• Clean, Current DEA Record (Required)
• COVID-19 Vaccination (Required)
• Overall knowledge of medical office patient care
• Knowledge of confidentiality issues involved with release of medical information
• Must have excellent interpersonal skills including ability to interact with staff and providers at other medical facilities
